NTSB issues its Accident Report on the Colgan Air DHC-8-402 N200WQ crash

Colgan Air’s (Continental Connection) (Manassas) captain of Colgan Air flight 3407 “inappropriately responded to the activation of the stick shaker, which led to an aerodynamic stall from which the airplane did not recover.” according to the National Transportation Safety Board (NTSB).

According to the NTSB; “On February 12, 2009, a Colgan Air, Inc., Bombardier DHC-8-400, (actually a DHC-8-402) registered N200WQ (msn 4200), operating as Continental Connection flight CO 3407, was on an instrument approach to Buffalo-Niagara International Airport, Buffalo, New York, when it crashed into a residence in Clarence Center, New York, about 5 nautical miles northeast of the airport. The 2 pilots, 2 flight attendants, and 45 passengers aboard the airplane were killed, one person on the ground was killed, and the airplane was destroyed by impact forces and a postcrash fire. The flight was a 14 Code of Federal Regulations (CFR) Part 121 scheduled passenger flight from Newark, New Jersey. Night visual meteorological conditions prevailed at the time of the accident.

The report states that, when the stick shaker activated to warn the flight crew of an impending aerodynamic stall, the captain should have responded correctly to the situation by pushing forward on the control column. However, the captain inappropriately pulled aft on the control column and placed the airplane into an accelerated aerodynamic stall.

Contributing to the cause of the accident were the Crewmembersโ€™ failure to recognize the position of the low-speed cue on their flight displays, which indicated that the stick shaker was about to activate, and their failure to adhere to sterile cockpit procedures. Other contributing factors were the captainโ€™s failure to effectively manage the flight and Colgan Airโ€™s inadequate procedures for airspeed selection and management during approaches in icing conditions.”

Southwest Airlines signs contract with Row 44 to equip the entire fleet with WiFi

Southwest Airlines (Dallas) has concluded its test phase and signed a contract with Row 44 to equip the entire fleet with WiFi.

Air India to trim its fleet by one-third to 105 aircraft

Air India (Mumbai) will trim its fleet from the current 146 to 105 aircraft by March 2011 according to the WSJ. This will include returning Boeing 777s and Airbus A319s and A320s coming off lease.

Southwest Airlines to start operations from Panama City, FL on May 23, 2010

Southwest Airlines (Dallas) has announced its schedules and fares from the new Northwest Florida International Airport near Panama City, FL. The new airport is set to open on May 23, 2010. WN will fly to Baltimore/Washington, Houston (Hobby), Nashville and Orlando. In addition the company has announced its summer 2010 schedules which will see a net gain of 41 roundtrips systemwide.

ATI’s pilots vote to join ALPA

ATI-Air Transport International’s (Little Rock) have voted to join the Air Line Pilots Association (ALPA) union.

USA 3000 Airlines returns to Detroit (Metro)

USA 3000 Airlines (Philadelphia), which suspended service at Detroit in April, has announced its return to DTW. Flights to Cancun, Mexico and Ft. Myers, will restart on November 21, 2009. Service to Punta Cana, Dominican Republic, begins again on November 29. USA3000 will also add flights on selected days this winter to Montego Bay, and Puerto Vallarta.

United’s N331UA to operate the last 737 flight

United Airlines (Chicago) will operate the last Boeing 737 revenue between Salt Lake City and San Francisco tomorrow (October 27, 2009) with 737-322 N329UA (msn 24149). The pictured 737-322 N331UA (msn 24192) has been selected to operate the last UA 737 flight after more than 41 years. N331UA will operate a “farewell flight” on Wednesday October 28, 2009 between Washington (Dulles) and San Francisco via planned extended stops at the Chicago (O’Hare), Denver and Los Angeles hubs. N331UA will depart IAD around 0600 and arrive at SFO around 1800 on the historic goodbye flight. Both airliners will ferry to Victorville, CA for storage the following day. This week we will have a special salute of 41 years of Boeing 737s at United Airlines in the Airliners Gallery.

Republic to acquire 10 ERJ 190s from US Airways

Republic Airways Holdings (Indianapolis) has now confirmed our earlier report. The holding company will acquire 10 Embraer ERJ 190-100 IGW aircraft (ERJ 190-100ARs) from US Airways. The first four (N961UW and N963UW have already left US) will enter service flying for its subsidiary Midwest Airlines (probably operated by Republic Airlines) in November and December. The other six will enter service in the first quarter of 2010 for its “branded operations” (believed to also include some flying for Frontier Airlines). Republic will apply $35 million it loaded to US Airways towards the purchase.

JetBlue Airways (New York-JFK) has confirmed the rumors – it will introduce a complete new livery tomorrow night (October 13) in Orlando.

Here is the message to the employees:

Over the years weโ€™ve made our mark on the industry with a distinctive look โ€“ uniforms, signage, and tailfin patterns like mosaic, stripes, harlequin and windowpane.
