SkyLease Cargo Boeing 747-400F N908AR is destroyed on landing at Halifax

Halifax Stanfield International Airport issued this statement:

At 5:05 a.m. today, a Boeing 747-400 SkyLease Cargo aircraft arriving from Chicago, Illinois, went off the end of Runway 14/32 upon its scheduled landing. Halifax Stanfield has activated its emergency plan and its well-trained and highly skilled emergency response team is responding to the incident along with local emergency responders including Royal Canadian Mounted Police (RCMP), Halifax Regional Police (HRP), Halifax Regional Fire & Emergency (HRFE), and Emergency Health Services (EHS).

The aircraft was carrying four (4) crew members who have been removed from the aircraft and transported to hospital for treatment of minor injuries.

The airfield has reopened. However, flight schedules continue to be significantly impacted. Passengers are advised to check with their airlines on the status of flights prior to coming to the airport.

Old Guysborough Road has been closed to local traffic between Oldham Road and Tower Access Road as part of the RCMP’s investigation into this matter. Traffic will be redirected.

Transportation Safety Board statement and update on the investigation:

On 7 November 2018, at about 0505 Atlantic Standard Time (AST), a Sky Lease Cargo Boeing 747 overran Runway 14 of the Halifax/Stanfield International Airport. The aircraft came to rest off the end of the runway. There were 4 crew members on board. The TSB is investigating.

What we know

  • On 7 November 2018, at about 0505 AST, a Boeing 747-400 cargo aircraft operated by Skylease Cargo overran Runway 14 of the Halifax/Stanfield International Airport, coming to rest approximately 695 feet, or 210 metres, off the end of the runway. The aircraft was on a flight from Chicago/O’Hare International Airport to Halifax Stanfield International Airport.
  • There were 4 crew members and no passengers nor any cargo on board at the time of the occurrence. Minor injuries to the 4 crew members were reported following the occurrence, and the aircraft was substantially damaged.
  • There was a strong westerly wind gusting at 18 knots, which would be approximately 33 mph. It was raining at the time of the occurrence. In more technical terms, according to METAR issued 7 minutes before arrival, the winds were 250 degrees, 11 gusting 18 knots. The ceiling was 1,200 feet above ground and visibility was 8 miles.
  • The aircraft struck a localizer antenna during the overrun. Its landing gear collapsed, two engines separated from the aircraft, and the remaining engines were substantially damaged. A small post-impact fire originated from the detached No. 2 engine which was lodged under the tail of the aircraft.
  • Runway overruns are a TSB Watchlist issue. Since 2013, there have been an average of 9 overrun accidents and incidents every year in Canada. These can lead to aircraft damage, injuries and loss of life. The consequences can be particularly serious when there is no adequate runway end safety area (RESA) or suitable arresting material (see the video).
  • The TSB has 4 outstanding recommendations aimed at reducing the number of runway overruns, and minimizing risks should an overrun occur.

Progress to date

The investigation team has conducted the following information-gathering work:

  • The flight data recorder, cockpit voice recorder, along with other systems aboard the aircraft that contain flight data are being recovered. These will be sent to the TSB’s Engineering Laboratory in Ottawa for analysis.
  • An initial examination of the accident site has been conducted.
  • A thorough examination of Runway 14/32 has been conducted.
  • The investigation team is in contact with the Skylease, the US National Transportation Safety Board (NTSB), the American Federal Aviation Administration (FAA), the manufacturers Boeing and Pratt & Whitney, the Halifax Airport Authority, NAV CANADA and Transport Canada.
  • Transport Canada has appointed a Minister’s Observer who will obtain factual information and advise the Department of any significant regulatory factors. He or she might also identify deficiencies that require immediate corrective actions and coordinate the required support to the investigation.
  • The TSB investigation team will be augmented with investigators from Head Office, Quebec and Western Regions, HumanFactors, Engineering Laboratory and Communications.

Next steps

In the coming days and weeks, investigators will do the following:

  • Download and analyze the data from the aircraft’s cockpit voice recorder and flight data recorder.
  • Conduct interviews with witnesses, air traffic control, and airport personnel and others who may provide additional information useful to the investigation.
  • Thoroughly review audio and radar data from NAV CANADA.
  • Gather and analyze all available information about the weather and runway surface conditions at the time of the occurrence.
  • Examine aircraft systems, review the aircraft maintenance records, pilot training, qualifications and proficiency records.
  • Review policies, operational procedures and regulatory requirements.
  • Examine the terrain at the end of the runway at Halifax/Stanfield Airport to determine what role it played in aircraft damage.

Communication of safety deficiencies

Aircraft accident investigations are complex and the TSB will take the time it needs to complete a thorough investigation. However, should we uncover safety deficiencies that present an immediate risk, we will communicate them without delay.

It is important not to speculate or draw conclusions about the causes of occurrences. Several factors usually contribute to an accident.



Lion Air Boeing 737-800 skids off the runway at Gorontalo

Lion Air flight  JT 892 from Makassar to Gorontalo with 174 passengers and seven crew members overran the runway at Gorontalo’s Djalaludin Airport, Indonesia Sunday (April 29) while attempting to land. The Boeing 737-800 (PK-LOO) was badly damaged. There were no reported injuries.

US-Bangla Airlines Bombardier Q400 crashes on landing at Kathmandu

US-Bangla Airlines (Dhaka) Bombardier DHC-8-402 (Q400) registered as S2-AGU with 67 passengers and four crew members crashed today (March 12) crashed on landing in Kathmandu and exited the runway coming to stop in a field. The flight (flight BS211) was arriving from Dhaka.

The death toll has risen to 49.

According to local media reports, the remaining injured passengers have been rescued and taken to local hospitals.

US-Bangla Airlines is a subsidiary of US-Bangla Group, a United States-Bangladesh joint venture company.

This is the second incident involving their Q400s. Previously on September 4, 2015, a Q400 skidded off the runway while landing at Saidpur Airport on a domestic flight. There were no injuries in that incident.

Bombardier issued this statement:

Everyone at Bombardier is saddened by the tragic event involving the Q400 aircraft. We extend our sympathies to the families of those whose lives were lost in this accident, and our thoughts are with those who survived.

Bombardier Air Safety Investigation Office is in contact with the authorities, and will provide assistance to all authorities as needed.

The Q400 aircraft has been designed to be robust and reliable in consideration of the high cycle demand of regional airlines and was designed to comply or exceed all airworthiness certification requirements. The Q400 fleet has accumulated over 8.5 million flight hours, and has performed about 10 million takeoffs and landings to date.

The Bombardier Q400 turboprop has been in service since 2000. Currently, there are over 500 Q400 aircraft in service worldwide, and the fleet dispatch reliability over the past year was consistently over 99.5%.

Belavia’s flight B2 8243 with EW-336PA leaves the pavement at Kiev

Belavia’s flight B2 8243 operating from Minsk to Kiev on December 22, 2017 reportedly with the pictured Boeing 737-3Q8 EW-336PA (msn 26312) reportedly left the payment after landing on runway 18L at Kiev. The passengers and crew members safely evacuated the aircraft. It is unclear at this time if the surface was the runway or the taxiway.

More information to follow. The airline has not put out a statement.

Kalstar Aviation (Serpong, Indonesia) Embraer ERJ 190-200LR (ERJ 195) PK-KDC (msn 19000057) while operating flight KD 678 from Denpasar (Bali) to Kupang, Indonesia, with 120 passengers and five crew members on board, overran runway 07 last night (December 21) while landing at Kupang. No injuries were reported according to the Jakarta Globe.

Dynamic International Airways issues an update on the FLL aircraft fire

Dynamic International Airways (Greensboro) has issued this follow-up statement:

“On behalf of Dynamic International Airways, I want to express our sincere apologies to our passengers as well as friends and families of the passengers on board flight DYA405 which experienced reported incident,” stated Don Dodson Director of Operations at Dynamic International Airways.

“Per our internal investigation, we would like to share following information:

After successfully passing required safety, mechanical and operation pre-flight inspections, Dynamic International Airways flight 2D0405 was taxiing and preparing for on time departure from Fort Lauderdale-Hollywood International Airport to Caracas Simon Bolivar Airport. Flight crew as well as airport personnel noticed engine problem during taxiing and acted according to emergency procedures. Passengers and flight crew were evacuated from the aircraft. Flight DYA405 was carrying 90 passengers as well as crew compliment of 11 people. Minor number of reportedly injured were treated and most of them were released shortly after. We are waiting for more detailed information from relevant parties in order to assure accuracy of the information we are providing. The experience of the Captain and First officer were 15700 and 4140 hours respectively. Per initial information the actions of the cabin crew were exemplary and according to their safety training.”

“Our primary focus is to provide support for the passengers who are at the airport and to any passengers who have been medically treated and/or released. Support received from airport authorities personnel as well as our colleagues from other airlines was tremendous and we would like to use this opportunity to express our gratitude for their help during and after the incident,” he continued.

Airline has provided Subservice for the recovery flights 2D 0405 and return flight 2D 0406 on 29th of October for all passengers impacted with today’s flight disruption. Flight 2D0405 is now departing on October 29th at 20:00 PM EST from Fort Lauderdale to Caracas, while Flight 2D0406 will depart Caracas to Fort Lauderdale at 02:00 AM local time on October 30th. Checks in counters are open for flight 2D 0405 while checks in counters for flight 2D0406 are opening at 21:00. Passengers are also able to receive vouchers, full refunds, and hotel accommodation in case they would like to travel on tomorrow flight as per their preference. Other flights were not impacted by the incident and operation is continuing according to schedule.”

“Dynamic is also in the process of dispatching a team to assist the National Transportation Safety Board in its investigation. We would not like to speculate about the potential causes of the incident during the ongoing investigation and all of us at Dynamic are committed to fully cooperating with the NTSB and other authorities. We have activated our Family Assistance Plan and have established a special toll-free number. Family members of the passengers who may have been on board Flight DYA405 may contact 1 (888) 283-2157 or 262-226-8787 for more information. Dynamic International Airways lead delegate Joseph Schoofs is at the airport in order liaise with passengers, while additional executive management team will be onsite by 2000 this evening,” he finished.
