Boeing statement on Lion Air flight 610 preliminary report

Delivered August 13, 2018, crashed into the Java Sea on October 29, 2018

Boeing issued this statement after the Indonesian government issued its preliminary report on the tragic crash of Lion Air flight 610 in the Java Sea:

The Boeing Company is deeply saddened by the loss of Lion Air Flight 610. We extend our heartfelt condolences and sympathies to the families and loved ones of those onboard.

Safety is a core value for everyone at Boeing and the safety of our airplanes, our customersโ€™ passengers and their crews is always our top priority. As our customers and their passengers continue to fly the 737 MAX to hundreds of destinations around the world every day, they have our assurance that the 737 MAX is as safe as any airplane that has ever flown the skies.

Boeing appreciates Indonesiaโ€™s National Transportation Safety Committee (NTSC) for its ongoing efforts to investigate the causes of the accident. Boeing is taking every measure to fully understand all aspects of this accident, working closely with the U.S. National Transportation Safety Board as technical advisors to support the NTSC as the investigation continues.

Earlier today, the NTSC released its preliminary accident investigation report.ย  The report provides detailed accounts of Flight 610 and of the immediately preceding flight of the same aircraft.

The report explains that the maintenance logs for the accident aircraft recorded problems related to airspeed and altitude on each of the four flights that occurred over the three days prior to Flight 610. The logs indicate that various maintenance procedures were performed, but issues related to airspeed and altitude continued on each successive flight. The logs indicate that, among other procedures, on Oct. 27, two days prior to the incident flight, one of the airplaneโ€™s Angle of Attack (AOA) sensors was replaced.

On Oct. 28, before the flight immediately prior to Flight 610, the pilot in command and the maintenance engineer discussed the maintenance that had been performed on the aircraft. The engineer informed the pilot that the AOA sensor had been replaced and tested. The report does not include records as to the installation or calibration of the new sensor, nor does the report indicate whether the sensor was new or refurbished.ย  Although the report states that the pilot was satisfied by the information relayed by the engineer that the AOA sensor had been replaced and tested, on the subsequent flight the pilots again experienced problems with erroneous airspeed data, and also experienced automatic nose down trim.

The report states that the flight crew of the Oct. 28 flight turned off the stabilizer trim switches within minutes of experiencing the automatic nose down trim, and continued with manual trim through the end of the flight. The report further notes that the pilot performed three non-normal checklist procedures, including the runaway stabilizer non-normal checklist, which is a memory item prescribed by the 737 MAX Flight Crew Operations Manual, and reaffirmed in Boeing Flight Crew Operations Manual Bulletin TBC-19 and FAA Emergency Airworthiness Directive (AD) Number 2018-23-51, as the appropriate procedure to address unintended horizontal stabilizer movement, regardless of source.

The report indicates that the remainder of the Oct. 28 flight was uneventful and that the flight continued to its destination. The report also states that, after landing, the pilot reported some of the experienced issues both on the aircraft maintenance log and to engineering. The report states that the pilot ran the runaway stabilizer non-normal check list, but it does not state that he communicated that fact in the maintenance documentation following that flight.

The following day, Oct. 29, shortly after taking off, the pilots experienced issues with altitude and airspeed data that the pilots had previously experienced on the earlier flights, due to erroneous AOA data. Data from the flight data recorder summarized in the report also makes clear that, as on the previous flight, the airplane experienced automatic nose down trim. In response, the flight crew repeatedly commanded nose up trim. This sequence repeated for the remainder of the flight, during which the flight crew was able to maintain control of the airplane for approximately ten minutes. Unlike as is stated with respect to the prior flight, the report does not state whether the pilots performed the runaway stabilizer procedure or cut out the stabilizer trim switches.

In accordance with international protocol, all inquiries about the ongoing accident investigation must be directed to the NTSC.

Fly Jamaica flight OJ256 crash lands at Georgetown

Crash landed at Georgetown, Guyana on November 9, 2018

Fly Jamaica flight OJ256 from Georgetown, Guyana to Toronto this morning returned to Georgetown due to a “technical problem”.

Ex-ATA Airlines Boeing 757-23N N524AT (msn 30233) was being operated by Fly Jamaica on the flight.

The crew declared an emergency. On landing at Georgetown Cheddi Jagan International Airport, N524AT suffered significant damage.

According to the airport, the flight was carrying 120 passengers and eight crew members.ย Six people were injured and are being treated at Diamond Hospital. Injures are not life threatening.

UTair flight UT579 overruns the runway at Sochi

UTair flight UT 579 from Moscow (Vnukovo) to Sochi with 164 passengers and six crew members, overran the runway on landing at Sochi. The left engine of Boeing 737-800 VQ-BJI caught on fire. The aircraft sustained significant damage and is probably an insurance write off.

All occupants were able to evacuate safely from the aircraft but 18 people reportedly received some injuries.

The airline issued this statement on social media (translated from Russian):

The Boeing 737-800 aircraft with the flight number UT579 Moscow – Sochi, after landing has gone beyond the runway. The aircraft went into the direction of the river, resulting in the collapse of the chassis and wing struts, the ignition of the left engine occurred.

There were 164 passengers and 6 crew members on board. The fire units of Sochi Airport have extinguished the fire, people have been evacuated. No casualties. The airline and aviation authorities have begun to investigate the causes of the incident.

US-Bangla Airlines Bombardier Q400 crashes on landing at Kathmandu

US-Bangla Airlines (Dhaka) Bombardier DHC-8-402 (Q400) registered as S2-AGU with 67 passengers and four crew members crashed today (March 12) crashed on landing in Kathmandu and exited the runway coming to stop in a field. The flight (flight BS211) was arriving from Dhaka.

The death toll has risen to 49.

According to local media reports, the remaining injured passengers have been rescued and taken to local hospitals.

US-Bangla Airlinesย is a subsidiary of US-Bangla Group, a United States-Bangladesh joint venture company.

This is the second incident involving their Q400s. Previously onย September 4, 2015, a Q400ย skidded off the runway while landing at Saidpur Airport on a domestic flight. There were no injuries in that incident.

Bombardier issued this statement:

Everyone at Bombardier is saddened by the tragic event involving the Q400 aircraft. We extend our sympathies to the families of those whose lives were lost in this accident, and our thoughts are with those who survived.

Bombardier Air Safety Investigation Office is in contact with the authorities, and will provide assistance to all authorities as needed.

The Q400 aircraft has been designed to be robust and reliable in consideration of the high cycle demand of regional airlines and was designed to comply or exceed all airworthiness certification requirements. The Q400 fleet has accumulated over 8.5 million flight hours, and has performed about 10 million takeoffs and landings to date.

The Bombardier Q400 turboprop has been in service since 2000. Currently, there are over 500 Q400 aircraft in service worldwide, and the fleet dispatch reliability over the past year was consistently over 99.5%.

Iran Aseman Airlines ATR 72 crashes in Iran, all on board dead

Airline Color Scheme - Introduced 1999

Iran Aseman Airlines flight EP 3704 from Tehran (Mehrabad) to Yasuj in southern Iran has crashed.

ATR 72-212 EP-ATS was operating the flight in bad weather with 60 passengers and six crew members. The aircraft disappeared from radar over the Zagroz Mountains. The wreckage has now been found at around 13,000 feet. There are no survivors.

All 66 passengers and crew were believed to have been killed in a plane crash in central Iran on Sunday, an airline spokesman said, after the domestic flight came down in bad weather in a mountainous region.

Operated by Iranian carrier Aseman Airlines, the plane crashed near the town of Semirom after taking off from Tehranโ€™s Mehrabad airport, news agency ISNA quoted emergency services spokesman Mojtaba Khaledi as saying.

The ATR-made plane was on a flight to the southwestern city of Yasuj and a spokesman for the airline said there were 60 passengers and six crew aboard.

โ€œAll…on board were unfortunately killed,โ€ Aseman spokesman Mohammad Taqi Tabatabai told state television.

Rescue helicopters were unable to land at the probable crash site because of heavy fog and emergency workers were searching the mountainous area by land, the television report said.

โ€œHeavy snow is making it difficult for rescuers to find the site of the crash,โ€ a television reporter accompanying rescue teams told state television.

Media reports said the twin-engined turboprop ATR 72 disappeared from radar screens 50 minutes after taking off from Mehrabad airport.

Supreme Leader Ayatollah Ali Khamenei and President Hassan Rouhani issued messages of condolences and the president asked the transport minister to lead an investigation into the crash.

Mehrabad is located in western Tehran and mainly functions as a domestic airport, although it also serves some international routes.

Iran has suffered several plane crashes in the past few decades. Tehran says U.S. sanctions have long prevented it from buying new aircraft or spare parts from the West. The crashed ATR was 25 years old, officials said.

Saratov Airlines Antonov An-148 crashes near Moscow, all dead

Saratov Airlines Antonov An-148 has crashed near Moscow after departing from Domodedovo Airport in snow with 65 passengers and six crew members on board.

The aircraft disappeared from radar after departure. The aircraft crashed 14 miles northeast of the airport.

The flight was headed to Orsk as flight 6W 703.

The aircraft involved is RA-61704.

There are no survivors.

Sydney Seaplanes DHC-2 Beaver crashes, 6 dead, operations suspended

Sydney Seaplanes de Havilland Canada DHC-2 Beaver, registration VH-NOO,ย crashed at Cowan Creek 30 km (18.5 miles) north of Sydney at about 3.15 pm local time on December 31, 2017. Five passengers and one crew member died in the crash.

Statement from Sydney Seaplanes regarding the aircraft incident at the Hawkesbury River, December 31, 2017

Yesterday there was a tragic accident involving one of Sydney Seaplanes aircraft in the Hawkesbury River, north of Sydney. As we know all six people on the flight (5 passengers and 1 pilot) have lost their lives.

All at Sydney Seaplanes are deeply shocked by this incident and the resulting loss of life. We wish to pass on our heartfelt condolences to the Bowden and Cousins families and the family of our pilot Gareth Morgan who were tragically killed.

Gareth had worked for Sydney Seaplanes on two occasions, the first from 2011 to 2014, after which he went to fly seaplanes in the Maldives. He then returned to working for us in May 2017. He was an extremely experienced pilot, with over 10,000 hours total time, of which approximately 9,000 hours was seaplane time. On a personal level he was deeply respected and liked by me and all of the team here as a man and as a pilot. He flew my family and I to Palm Beach just before Christmas.

I have spoken to Garethโ€™s parents, who live in Canada and offered our deepest sympathies and we will support them in any way we can.

We are devastated by both Garethโ€™s loss and that of the Bowden and Cousins families.
I would like to take this opportunity to thank the NSW Police and all that were involved in the recovery yesterday, this would have been a harrowing task and we are extremely grateful to them for their work.

Nature Air Cessna Grand Caravan crashes in Costa Rica, 12 dead

Nature Air Cessna 208B Grand Caravan registered as TI-BEI (msn 208B0900) crashed on takeoff from Punta Islita, Costa Rica on December 31, 2017 with 10 passengers (all from the USA) and two crew members. The aircraft crashed and burned in a wooded area. All on board died in the crash. The aircraft was bound for San Jose. TI-BEI was painted in one of the distinct original art tails (Sergio Morera):

Starbow’s first ATR 72-500 veers off the runway at Accra, Ghana, suspends operations

Starbow Airliners took delivery of its first ATR 72-500 aircraft, the pictured 9G-SBF, on November 22, 2017. The second ATR 72 aircraft is scheduled to arrive soon.

However three days later on November 25, 2017, the new type was involved in an runway accident at Accra, Ghana while operating flight S9 104.

Reportedly, the aircraft with 63 passengers and five crew members veered off the runway in heavy rain while attempting its takeoff and exited the runway coming to rest in the perimeter fence.

Starbow Airlines issued this statement:

Starbow S9 104 Incident at Accra, Kotoka International Airport.

Everyone at Starbow wishes to express their sincere regret for the distress experienced by our guests on Flight S9 104 on November 25, 2017 at 12:45, local time. The service, operated by ATR 72-500 (registration 9G-SBF), was a scheduled flight from Accra to Kumasi. All sixty-five guests and five crew disembarked, as normal, though five guests were treated for minor injuries at the Airport Clinic. The Ghana Civil Aviation Authority has begun an investigation into the incident.

Eric Antwi, Chief Executive Officer, said: โ€˜All of us at Starbow at this time remain dedicated to our guests, our crew and their families. Their safety and wellbeing is at the heart of everything we do, and we will cooperate fully with the Ghana Civil Aviation Authority, and other agencies, to determine the cause of this incident. We hope to be able to provide further information in due courseโ€™.

Starbow has suspended its operations with immediate effect and guests with future reservations are invited to contact the airline at 024 500 0000 to make alternative travel arrangements.

West Atlantic’s CRJ200 SE-DUX crashes in the border region of Sweden and Norway, 2 crew members dead

West Atlantic Cargo Airlines (West Air Europe-Sweden) (West Atlantic Group) (Gothenburg, Sweden) Bombardier (Canadair) CRJ200 (CL-600-2B19) with the registration of SE-DUX (msn 7010) late on January 8 with two pilots was operating flight SWN 294 between Oslo and Tromso, Norway, crashed in the mountainous border region of Norway and Sweden. The crew members declared an emergency and disappeared from radar. The jetliner impacted a mountain northwest ofย Lake Akkajaure in Lapplandsfjรคllen near the Norwegian border. There were no survivors.

West Atlantic was founded in 1962 and is headquartered in Gothenburg, Sweden. Operations are performed all over Europe.

The aircraft departed Oslo on route to Tromsรถ and declared mayday at 23:31 (January 8) whereby the Swedish and Norwegian search and rescue teams were notified. The crash site was located at 03:10 (January 9) near the Norwegian border by the Lake Akkajaure in the Swedish Lapplandsfjรคllen by air rescue services with support from Hovedredningssentralen in Norway.

The search has been taken over by the Swedish police which are on their way to the accident site. The internal process is coordinated by the Companyโ€™s Emergency Response Team.
